4. Scheduling & Attendance
Appointments are scheduled in advance. You agree to:
Arrive on time for your appointment
Understand that late arrival may result in a shortened session
5. Cancellation & No-Show Policy
We require at least 24 hours notice for cancellations or rescheduling.
One late cancellation (less than 24 hours) may be forgiven
Additional late cancellations may be charged the full session fee
Missed appointments may result in limited or discontinued scheduling privileges
These fees are the patient’s responsibility and are not billable to insurance.
6. Payment Policy
Payment is due at the time services are rendered unless otherwise arranged.
By scheduling services, you agree that:
You are financially responsible for all charges incurred
You may be required to maintain a valid card on file
Mend Clinic may charge your card for services and applicable cancellation fees

We do not guarantee reimbursement
You are responsible for verifying benefits, obtaining referrals, and submitting claims if desired

12. Direct Access (North Carolina)
Patients may receive physical therapy without a physician referral under North Carolina law. However, referral to a physician may be required if progress is not observed within a legally defined timeframe.
13. Good Faith Estimate
Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges.
If your bill exceeds this estimate by $400 or more, you may dispute the charge.
